Please complete the form below and clearly state the medication that requires a refill in the space provided.

Please Note:

  • Medication refills require approval from one of our veterinarians.

  • For legal compliance, we must have examined your pet within the last 6 months.

  • Please allow 1-2 days for our staff to consult with the veterinarian and prepare your refill request.

  • Special order medications may take 3-5 business days to process.

  • A fee applies to all written prescriptions.

  • Medication refills are not available for patients who have not been assessed by our clinic.
